This is to draw your kind attention to the published Government directive on Covid-19 vaccination of school going students aged between 12 and 17 years.
Please note that we have been informed to provide details of the students who are willing to take the vaccine arranged by the Government by on or before 18 October 2021.
Please click on the following google form link to provide the correct and appropriate information necessary for the registration process. Kindly note that any incorrect information provided, may cancel the enrollment process i.e. a wrong digit in the birth registration number or wrong birth date etc. You are requested to fill it up cautiously and check it before sending it. Also, the birth registration number (17 digits) will be needed to fill-up the form.

This information will be shared only with the concerned Government Office.
